Documentation Guidelines

The DRC at Southern Utah University follows ADA (American with Disability Act) mandates. To provide any accommodations, we need formal documentation done by a professional stating the following:

  1. Diagnosis using the respective criteria depending upon the type of disability
  2. Substantial Limitation in major life activities, limiting the individual's learning and access to the classroom. These limitations must be personal and specific.

You may hand deliver documentation to our office at 黑料天堂 in the Sharwan Smith Student Center room 206 or send it through our secure file upload system to one of our staff members

Disability Support Eligibility

We serve students with disabilities as defined by the Americans with Disabilities Act. Some of these qualifications include, but are not limited to:

  • Cancer
  • Diabetes
  • Epilepsy
  • Cerebral Palsy
  • Multiple Sclerosis
  • Heart Disease
  • Psychiatric Disability
  • Specific Learning Disability
  • Traumatic Brain Injury
  • Orthopedic/Mobility Impairments
  • Speech/Language Impairments
  • Blindness/Visual Impairments
  • Deafness/Hearing Impairments
  • ADD/ADHD
  • Dyslexia

Support Eligibility for Other Conditions

We also support students with certain temporary medical conditions that may limit or impact their participation in classes or University activities for an extended period. These short-term conditions may include pregnancy, pregnancy-related complications, childbirth, as well as a significant surgery or injury. These types of medical conditions may qualify a student for short-term adjustments or accommodations.
